The Community Transformation Grants (CTG) are administered by the Centers for Disease Control and Prevention and funded through the Prevention and Public Health Fund (PPHF). A CTG aims to improve community health through community partnerships and community-based interventions. It is expected that the implementation of a CTG will improve the health of every 4 out of 10 in your community.
