DESCRIPTION
More people than ever are surviving cancer. With that news comes the need for physicians to have a model of care that ensures quality of life beyond cancer. This course will bridge the gap between Oncology and Primary Care to enhance the quality of survivorship.
The intent is to educate healthcare professionals in the components of a survivorship care plan, to provide an overview of the management and surveillance of late effects of cancer and treatment, and to identify psychosocial challenges that affect survivorship in the midst of other chronic conditions.
TARGET AUDIENCE
Internists, General Practitioners, Family Physicians, Oncologists, Nurses, Social Workers, and other healthcare providers practicing in primary care, oncology and public health settings.
COURSE OBJECTIVES
After completing this learning activity, the participant will be able to do the following:
- Describe the benefits of a survivorship care plan on quality of care for the cancer survivor.
- Discuss the management of chronic conditions resulting from cancer and treatment.
- Improve the surveillance of late effects and potential second primary cancers.
- Identify quality of life issues and interventions for survivors.

Participants claiming CME credit from this activity may submit the credit hours to the American Osteopathic Association Council on Continuing Medical Education for Category 2 credit.
The Ohio Board of Nursing will accept at face value the number of contact hours awarded for an educational activity that has been approved for CE by a nationally accredited system of CE approval. Therefore, the 6.25 physician AMA PRA Category 1 Credits™ will be accepted for nursing credits. Nurses from other states should contact their State Board of Nursing.
This activity has been approved for 5.6 Social Work CEUs.

Pre-registrations are accepted by fax or online until 4:00 pm ET, April 24, 2009. To register after this date you must do so on-site.
In case of cancellation, a full refund will be made if cancelled by April 24, 2009. After April 24, 2009, a $50 cancellation fee will be deducted from your refund. Written notification of your cancellation is required to process your refund.
NO REFUNDS WILL BE ISSUED AFTER MAY 29, 2009.
